Safety for the commercial driver and the public is the primary objective of the Federal Motor Carrier Safety Administration(FMCSA). The Federal Motor Carrier Administration mandates companies with the following criteria, provide a drug and alcohol testing program:
- Drives a vehicle of over 26,001 GVWR or
- Drives a tow truck with combined GVWR rating of 26,001 lbs. or more,
- Inclusive of a towed unit with a GVWR or more than 10,000 lbs. or
- Drives a DOT placarded vehicles under hazardous material regulations or
- Drives a vehicle designed to transport 16 passengers or more.

A DOT physical is a medical examination required by the Federal Motor Carrier Safety Administration (FMCSA) for all commercial motor vehicle (CMV) drivers. The exam ensures drivers are physically and mentally capable of safely operating commercial vehicles. It must be performed by a medical examiner listed on the FMCSA National Registry of Certified Medical Examiners.

The DOT physical includes a comprehensive health history review, vision test (must have at least 20/40 acuity in each eye), hearing test (must perceive a forced whisper at 5 feet), blood pressure and pulse check, urinalysis for underlying medical conditions like diabetes, and a thorough physical examination of your heart, lungs, spine, and extremities. The examiner will also evaluate your neurological function and check for hernias.
A DOT medical card is typically valid for up to 24 months. However, if you have certain medical conditions such as high blood pressure, diabetes, or other health issues that require monitoring, the medical examiner may issue a card valid for a shorter period — sometimes as little as 3 months — with a requirement to return for re-evaluation.
No. The DOT physical and the DOT drug test are two separate requirements. The urinalysis during the physical checks for medical conditions like diabetes — not for drugs. However, FMCSA regulations require a pre-employment drug test before a driver can operate a CMV, and drivers must also be enrolled in a random drug testing program.
