Step 1: SAP Evaluation

Get matched with a DOT-qualified Substance Abuse Professional (SAP) who evaluates your situation and creates a personalized treatment or education plan.

After a DOT drug or alcohol violation, federal regulations require you to complete an evaluation with a qualified SAP before you can return to safety-sensitive work. The SAP will assess your situation, determine whether you need education, treatment, or both, and develop a plan tailored to your needs. The SAP evaluation is the critical first step — without it, you cannot legally proceed with return-to-duty testing.

Step 2: Complete Treatment & Education

Follow the SAP's recommended treatment or education program, then pass a follow-up SAP evaluation confirming you're ready to return to duty.

Once your SAP prescribes a treatment or education program, you must complete it in full before moving forward. This may include counseling sessions, substance abuse education courses, or a structured treatment program — depending on your evaluation results. After completing the program, you’ll return to your SAP for a follow-up evaluation. The SAP will determine whether you’ve successfully met the requirements and are eligible to proceed to the return-to-duty test. This step ensures you’re fully prepared and compliant with DOT/FMCSA regulations before testing.

The final step is the return-to-duty test itself. DOT regulations require a directly observed drug test (DOT 5-panel) and/or an alcohol test with a result below 0.02 BAC. Directly Observed Drug & Alcohol Testing

All DOT return-to-duty drug tests are conducted under direct observation as required by 49 CFR Part 40. We schedule your test at one of 20,000+ SAMHSA-certified collection sites nationwide. Drug testing uses the standard DOT 5-panel urine test ($69), and alcohol testing uses an evidential breath testing device ($59). Results are reviewed by a licensed Medical Review Officer (MRO) and reported within 24–48 hours.

Follow-Up Testing Program

After you pass your return-to-duty test, the SAP prescribes a follow-up testing schedule — a minimum of 6 unannounced tests in the first 12 months, with the possibility of testing for up to 60 months. FMCSA – Federal Motor Carrier Safety Administration-DOT Mandated Compliance
Safety for the commercial driver and the public is the primary objective of the Federal Motor Carrier Safety Administration(FMCSA). The Federal Motor Carrier Administration mandates companies with the following criteria, provide a drug and alcohol testing program:
A truck driver that holds a Commercial Driver’s License and:
  • Drives a vehicle of over 26,001 GVWR or
  • Drives a tow truck with combined GVWR rating of 26,001 lbs. or more,
  • Inclusive of a towed unit with a GVWR or more than 10,000 lbs. or
  • Drives a DOT placarded vehicles under hazardous material regulations or
  • Drives a vehicle designed to transport 16 passengers or more.
